Admissions Eligibility Criteria
C.H.O.I.C.E., Inc. quality residential treatment program admits male youth ages 13 to 17 years old who have moderate to severe emotional and/or behavioral problems that interfere with their daily functioning at home, foster, or adoptive placement.The common problems include:
- Depression or Dysthymia
- Anxiety Disorder
- Disruptive Mood Dysregulation
- Disruptive, Impulse-Control, and Conduct Disorder
- Substance Abuse
- Post-traumatic Stress Disorder
- Trouble With the Law
- Unresolved issues related to histories of abuse, neglect, and/or exposure to family conflict
- Unresolved feelings about rejection or loss related to adoption, family separation, death, and/or abandonment by caregivers
There are Five Easy Steps to submit an application for admission
Step 1– Contact Us: Simply fill out the online Referral Form
or call us directly at 813-324-8547. We will contact you within 48 hours to discuss our program with you and help you determine if our program is an appropriate placement for your male youth.
Step 2– Pre-screening: A preliminary screening interview will be conducted with an intake staff to determine if the youth meets the admission eligibility based on the following indicators:
- Presenting Problem
- Diagnosis
- The current level of functioning
- Necessity for this level of services
Step 3– Complete Application: An application is provided after you have completed the preliminary screening and the youth has met the eligibility criteria for this level of services. Once we receive the application it will be reviewed by an intake staff. The application can be emailed, faxed, or post mailed to us.
Step 4– Clinical Interview: A decision on whether a youth is a good fit for C.H.O.I.C.E., Inc. program will be decided after the youth, family, and/or guardian participate in a required face-to-face interview prior to us making a final decision. You should plan at least 2 hours for the interview. A decision is made within one business day (usually within a few hours). If the youth is accepted, we will contact you to work out the final items and details for his admission into the program.
Step 5– Intake Process: There is additional forms and paperwork that will need to be completed, therefore you should allow at least two hours for this process.

- Medications
